The Hospital Case Manager plays a crucial role in ensuring that patients receive high-quality, efficient, and well-coordinated care throughout their hospital stay.
Conduct comprehensive assessments of patients' medical, social, and psychological needs to develop individualized care plans.
Collaborate with the healthcare team to coordinate patient care, including medical treatments, consultations, and discharge planning.
Ensure that healthcare resources are used efficiently and that patients receive appropriate levels of care.
Advocate for the needs and concerns of patients and their families, helping them navigate the healthcare system.
Develop and implement discharge plans to ensure a smooth transition from hospital to home or other care settings.
Maintain accurate and detailed records of patient assessments, care plans, and interventions.
Participate in quality improvement initiatives to enhance patient care and outcomes.
Foster effective communication and teamwork among healthcare professionals to ensure a collaborative and patient-centered approach.
Provide education and support to patients and their families to enhance their understanding of the care plan and condition management.
Ensure that all care coordination activities adhere to regulatory and accreditation standards.
B Degree/Diploma Nursing qualification or equivalent NQF level 6 is preferred.
Enrolled nursing qualification or equivalent NQF level 4 is essential.
Qualification in associated Healthcare sciences or Managed care/ Case management.
3 years professional healthcare experience in hospital.
Intensive Care Nursing experience is essential
Previous hospital and medical scheme case management experience is essential
Computer literate & ICD 10 coding
