Hospital Case Manager

As the leading medical recruitment agency in South Africa we have an exciting opportunity for a Hospital Case Manager based in Durban to coordinate and optimize patient care and outcomes. The Hospital Case Manager plays a crucial role in ensuring that patients receive high-quality, efficient, and well-coordinated care throughout their hospital stay.


Responsibilities:

  • Conduct comprehensive assessments of patients' medical, social, and psychological needs to develop individualized care plans.

  • Collaborate with the healthcare team to coordinate patient care, including medical treatments, consultations, and discharge planning.

  • Ensure that healthcare resources are used efficiently and that patients receive appropriate levels of care.

  • Advocate for the needs and concerns of patients and their families, helping them navigate the healthcare system.

  • Develop and implement discharge plans to ensure a smooth transition from hospital to home or other care settings.

  • Maintain accurate and detailed records of patient assessments, care plans, and interventions.

  • Participate in quality improvement initiatives to enhance patient care and outcomes.

  • Foster effective communication and teamwork among healthcare professionals to ensure a collaborative and patient-centered approach.

  • Provide education and support to patients and their families to enhance their understanding of the care plan and condition management.

  • Ensure that all care coordination activities adhere to regulatory and accreditation standards.

Requirements:

  • B Degree/Diploma Nursing qualification or equivalent NQF level 6 is preferred.

  • Enrolled nursing qualification or equivalent NQF level 4 is essential.

  • Qualification in associated Healthcare sciences or Managed care/ Case management.

  • 3 years professional healthcare experience in hospital.

  • Intensive Care Nursing experience is essential

  • Previous hospital and medical scheme case management experience is essential

  • Computer literate & ICD 10 coding
