The Clinical pharmacist will be responsible for the provision of medication and supplementary services to patients and members of the healthcare team.
The incumbent will also assume responsibility for meeting patients’ needs in a responsible, satisfactory and cost-effective manner
Implement and maintain pharmaceutical evaluation (clinical ward round) of hospital In-patients as per Our client Hospital Clinical Ward Round specifications, with special care to geriatrics, paediatric, neonates, ICU and high care patients
Complete clinical ward rounds regularly in accordance with hospital needs and resources available
Document and capture all ward round queries
Load clinical pharmacist notes on the Bluebird programme daily.
Ensure pharmaceutical evaluation (clinical ward round) with respect to individualised medication regimens, continuous therapeutic drug monitoring, clinical intervention, pharma co-dynamic and pharmacokinetic consultation
Clinical ward rounds should include special focus on Antibiotics usage, high alert drugs and feeds
Clinical ward rounds must also assist and offer advice on the usage, side effects, contra-indications and storage of medication to patients and other members in the healthcare team
Ensure that the whole medication safety portfolio are implemented, sustained and improved to include all new associated projects in all wards and pharmacy
Quality medication rounds done in all wards and pharmacy as per criteria
Dispensing in and out patient prescriptions.
Provision of a drug information service
Assisting with the quality and clinical initiatives
To assist with training of Pharmacist Assistants and Interns.
Implementation of controls to enforce GPP and SOPs to minimize risks.
Maintaining effective relationships with key role players.
Communicating professionally and handling queries as per policy.
Assisting with stock take, stock control and expired stock removal from shelves.
Responsible for Balancing of S6 book in March, June, September and December and all scripts and invoices documentation and noted.
Tidiness of Pharmacy.
To be on call as per roster
Rotation between retail and hospital
Rotation on all tasks involved with Schedule 5&6 medication, including delivering to wards.
Will be required to work certain week ends and public holidays (as a dispensing pharmacist) - will be overtime (as required for roster and amount of pharmacists available
Assist with all aspects of ethical stock control including: re-ordering of stock, maintaining dedicated stock areas for organisation and expiry dates and conducting cyclical and full stock takes
